Advocacy – Ensuring that client’s needs are met

Crisis Intervention – Providing immediate guidance to client and/or family in identifying options available in their specific situation

Home Safety Evaluation – Comprehensive review of home for potential safety concerns

In Person Visits – visits with client and/or family members to determine care needs

Monitoring and coordination – work in conjunction with physician, home health, hospice and other care provides

Nursing Home/Assisted Living Placement assistance – educating client and/or family of options available and accompanying on tours as requested
